The Role:

As an Occupational Therapist, you will deliver home-based assessments and interventions to aged care clients throughout the Midwest region. You will be responsible for recommending assistive technologies, home modifications, and personalised therapy plans. This role offers a balance between remote working and in-home client visits, with the backing of a skilled and collaborative team.

Key Responsibilities:

  • Conduct person-centred assessments to identify functional capacity, strengths, and lifestyle goals.
  • Provide expert advice on assistive technology and home modifications tailored to individual needs.
  • Develop holistic, personalised therapy plans in partnership with clients and their carers.
  • Prepare professional reports and funding applications for assistive technology and home modification services.
  • Coordinate referrals, service delivery, and follow-up evaluations.
  • Collaborate with carers, health professionals, and service providers to ensure optimal outcomes for clients.

Essential Criteria:

  • Degree in Occupational Therapy and current AHPRA registration.
  • At least 12 months of experience, preferably in the aged care sector.
  • Expertise in assistive technology and home modifications, with a focus on person-centred care.
  • Strong communication and problem-solving skills, with the ability to manage time effectively.
  • Ability to work both independently and within a multidisciplinary team.
  • Valid driver’s licence and willingness to travel for client appointments.
