The Role:

As a Case Manager in the Out of Home Care Intensive Therapeutic Care Program, you’ll play a vital role in supporting young people aged 12-24 who are at risk or experiencing homelessness.
You’ll manage a caseload of up to 4 young people with complex needs, creating individualised case management and exit plans in line with best practice standards. Your ability to foster a trauma-informed, therapeutic environment will be key to helping young people build resilience, develop life skills, and progress toward independence.
In this role, you’ll work closely with internal teams, external stakeholders, and the young people themselves to coordinate care, facilitate case conferences, and advocate for their needs. You’ll also participate in outdoor education programs, house celebrations, and a rotating on-call roster. The position requires a deep commitment to child safety, an unwavering dedication to positive outcomes, and the emotional intelligence to navigate challenging situations with empathy and professionalism.

Essential Criteria:
  • Degree in social work, psychology, or relevant social sciences desired or a diploma in relevant field
  • Minimum 1-2 years of case management experience in Out of Home Care in Australia
  • Very strong Admin skills
  • Understanding of trauma-informed care and therapeutic frameworks
  • Experience working with young people with complex needs
  • Strong skills in negotiation, conflict resolution, and problem-solving
  • Knowledge of legislative and accreditation requirements for residential care
  • Ability to work within a rotating on-call roster (1 week every 10-12 weeks)
  • Current Working with Children Check and valid Driver’s License
  • First Aid Certificate or willingness to obtain one
