The Role:

As a Care Manager, you will lead a team dedicated to enhancing the quality of client care in the aged care remit. You will oversee case management services, ensuring person-centered support that aligns with aged care standards and best practices. Your role includes conducting assessments, developing care plans, and coordinating services to promote clients' independence and wellbeing. You'll collaborate with internal teams, external providers, and stakeholders to deliver high-quality home care services. Compliance and continuous improvement will be key priorities, as you contribute to audits, training, and quality assurance initiatives. Additionally, you will provide leadership and mentorship to case managers, fostering a culture of professional growth and excellence. Business development will also be part of your role, identifying opportunities to enhance services and build community partnerships. Your ability to navigate funding models, manage performance, and uphold clinical governance will be crucial to success.

Essential Criteria:

  • Diploma in Community Services, Health Sector Management, Aged Care, or a clinical background
  • Minimum 5 years of relevant industry experience
  • Strong knowledge of Aged Care reforms, standards, and sector requirements
  • Experience in case management within the home and community care sector
  • Proven leadership in service development and quality improvement
  • Excellent interpersonal and communication skills
  • Experience working with diverse groups (e.g., Aboriginal and Torres Strait Islander, CALD, Veterans, Disability, Mental Health, LGBTI+, and Homelessness)
  • Open Driver’s License and NDIS Worker Screening
  • Current First Aid and CPR certification
