The Role
Our client is seeking a dedicated Care Coordinator to oversee the delivery of high-quality disability support services to NDIS clients. This position will involve working closely with Directors, the Service Manager, and administrative staff to ensure that all clients receive the personalised care they need. The successful candidate will lead, supervise, and mentor support workers, develop and implement individualised support plans, and liaise with clients, families, and other stakeholders.
Key responsibilities include:
- Coordinating and managing the delivery of disability support services for NDIS clients
- Leading and supervising support workers across multiple homes
- Developing tailored support plans and ensuring smooth service delivery
- Monitoring client progress and adjusting plans as needed
- Mentoring new and existing staff members to maintain high service standards
- Collaborating with the team to drive continuous improvement
- Fulfilling requirements related to reportable incidents and restrictive practices
Essential Criteria:
- Relevant qualifications and experience in the Disability Support Services sector
- Previous experience in a similar or related role
- Strong communication and interpersonal skills with the ability to build rapport with clients
- Excellent organisational and time management skills, with the ability to prioritise and multitask
- A genuine passion for making a positive difference in the lives of individuals with disabilities
- Familiarity with the NDIS and its associated processes and requirements
- Current driver's licence and access to a reliable vehicle is required
