Responsibilities include developing care plans, monitoring adherence, evaluating effectiveness, tracking client progress, and facilitating necessary changes. The therapist will ensure efficient and effective use of resources by prioritising services for clients with the highest needs. Collaboration with the multi-disciplinary team and senior management is essential to enhance care planning and continuity across service boundaries.

Essential Criteria:
- Fully qualified Occupational Therapist, AHPRA Registered.
- Experience working in a similar role is desirable
- Knowledge of disability & mental health services.
- National Police Clearance, NDIS Screening, WWCC
- Driver’s Licence
