A well-known home care NFP are looking for a Clinical Care Partner to join their growing team in Albany.

The Role

The role will involve supporting clients across Support at Home, Fee for Service, and Brokerage programs. The role involves developing and reviewing care plans, providing clinical oversight, and delivering nursing interventions in line with legislation, quality standards, and client preferences. You will liaise with healthcare providers to ensure holistic care, manage client budgets, and accurately document and report care. A focus on continuous improvement, compliance, and safe practice is essential, along with participation in the on-call roster.

Requirements
  • Current unrestricted AHPRA Registration
  • Experience in aged care, community or disability
  • Excellent clinical and communication skills
  • Ability to work both autonomously and as part of a team
  • Police Check within 3 months, current drivers’ licence and full Australian working rights
